Daily Medication Form
Use this form if your child will need any medication administered during the school day. Parent/Guardian must bring the medication and Physician's orders to the nurse. An adult must pick up any unused medication at the end of the school year, or it will be discarded. New orders are needed each school year.

Emergency Medical Orders and Health Care Plan for Epipen/Benadryl Administration
Use this form if your child has significant allergic reactions. Parent/Guardian must bring the medication and Physician's orders to the nurse. An adult must pick up the medication at the end of the school year, or it will be discarded. If your child has been trained to self administer his medication, he/she may pick up their medication at the end of the school year. New orders are needed each school year.

Universal Physical Form for New Students
A current physical (within one year of registering ) is required for new students registering from another state or country. A student registering from another district in New Jersey must provide a copy of a physical. A copy of current immunizations is required of all new students.

Asthma Action Plan
Use this form if your child has Asthma and may need an inhaler or nebulizer treatment during school hours. If allowed by your Physician, your child may carry and /or self- administer their inhaler (name on inhaler please). However, the nurse must still have Physician's Orders for the use of the inhaler.

Scoliosis Screening Program
Students in 5th and 7th grades are screened for scoliosis unless the student has submitted proof of a current physical examination, or the parent/guardian requests that the student not be screened.
