4th Grade Recognition
Memory Book Questionnaire
Please return no later than December 18th, 2009
The memory book that you ordered for your child will be distributed at the 4th Grade Recognition Kid’s Luncheon on June 17th, 2010.
Please provide us with the following information which will be included with your child’s picture in the memory book.
1. NAME: ____________________________________________________________________
(Please print your child’s name as you would like it to appear in the memory book. Example: Jen Smith, Jenny Smith, Jennifer M. Smith, etc.)
2. MY FAVORITE COLES SCHOOL MEMORY: ____________________________________________________________________
This is to be completed by your 4th grader. Please print carefully and be specific. Use full sentences. Entry may be more than one but less than three sentences in length. (Examples: Field Day, being in the variety show, a class trip, Halloween parade, kickball at recess, class play, etc.).
*PLEASE NOTE: If you would prefer to e-mail your entry, please include the above information in the body of your e-mail. It is very important that in the subject line you address it: “Memory Entry for (your child’s name)” so that it gets sorted correctly. E-mail all entries to stacey_pritchett@yahoo.com.
Thank you in advance for helping your child with this information and completing it by December 18th. If sending in with your child, please place in an envelope marked “4th Grade Recognition”.
