Use the Deposit Form to submit checks, cash and funds received towards PTA activities.  Please include detail and total of the amounts submitted for deposit.  Return completed form and monies to the PTA Treasurer.

Use the Check Request form to request reimbursement for eligible expenses.  Copies of receipts need to be attached to the Check Request form.  Return completed form and receipts to the PTA Treasurer.
